The Occupational Safety and Health Administration has opened an investigation into the accident on the set of the Amazon MGM Studios film The Pickup. A U.S. Department of Labor spokesperson told The Hollywood Reporter early Wednesday that an inquiry had been initiated into the incident. “As the investigation is open, we are unable to provide preliminary or interim information at this time,” the spokesperson added. The accident in question occurred on a second unit set of the Atlanta-based film on Saturday and injured multiple crewmembers, an Amazon MGM Studios spokesperson confirmed on Tuesday. Crewmembers were shooting an action sequence that had previously been rehearsed and “did not go as planned,” the spokesperson stated. “We are still in the process of gathering facts on what happened and why, but first and foremost our thoughts are with those who are recovering.” The Associated Press reported on Tuesday that the injuries occurred after a truck being used in the sequence “locked up” and hit another car. “It was a completely freak accident,” one anonymous source told the AP. …

SINGAPORE: A 42-year-old car driver will be charged in court on Thursday (Apr 25) with four offences over his involvement in Monday’s multi-vehicle accident in Tampines that killed two people.  The offences under the Road Traffic Act include dangerous driving causing death and dangerous driving causing hurt, the police said late on Wednesday. He will also be charged with dangerous driving and failing to stop after an accident.  Police investigations are ongoing for other potential offences. The offence of dangerous driving causing death carries a jail term of between two and eight years, as well as disqualification from driving all classes of vehicles. The offence of dangerous driving causing hurt carries a fine of up to S$10,000, or jail for up to two years, or both, as well as disqualification from driving all classes of vehicles. Those found guilty of dangerous driving face a fine of up to S$5,000, a jail term of up to 12 months, or both. SINGAPORE: A 26-year-old male motorcyclist was killed in a road traffic accident on Wednesday (Apr 24) morning along the Pan Island Expressway (PIE). The police were alerted to an accident involving a lorry and a motorcycle at about 10.15am, they said on Wednesday in response to CNA’s queries. The Singapore Civil Defence Force (SCDF) told CNA it was alerted to the accident, which happened along PIE towards Kranji Expressway (KJE) after the Jalan Bahar exit, at about 10.20am. A 26-year-old male motorcyclist was pronounced dead at the scene by an SCDF paramedic, it added. Investigations are ongoing.  Source link

A group of amusement park visitors were injured Saturday evening after a Universal Studios tram accident in Los Angeles, California. The Los Angeles County Fire Department was dispatched to the scene shortly after 9 p.m. P.T. Saturday night and transferred 15 patients to local hospitals with minor injuries. One person was critically injured, NBC Los Angeles reported. A spokesperson for Universal Studios Hollywood confirmed the accident in a statement but provided no details to the circumstances. “We are working to support our guests and understand the circumstances that led to the accident,” the spokesperson said. Universal Studios is owned by NBC News’ parent company, NBCUniversal. California Highway Patrol is leading the investigation into the incident. A fatal accident inquiry is to be held into the death of a pedestrian who was hit by an Edinburgh tram on his way home from work. Bus driver Carlos Hernan Correa Palacio, 53, died after he was struck at a crossing in the Saughton area of the capital on 11 September 2018. Edinburgh Trams Limited (ETL) was fined £240,000 last year after admitting a breach of health and safety legislation over the incident in a hearing at Edinburgh Sheriff Court. The court heard how the driver sounded his bell a number of times and began to slow the tram down when he spotted Mr Correa. He applied the emergency brake, which also set off the tram’s warning horn, but Mr Correa was killed. The Crown Office said there were no issues with the tram driver and he had responded to the situation in accordance with his training. The tram was in working order and the braking system functional.
